Purpose: These rules (the “Rules”) govern the “Avéli^® The Naked Truth Patient Offer” (the “Offer”). By requesting the offer, you agree to be bound by these Rules.

Sponsor: The Sponsor of the Offer is Revelle Aesthetics, 2570 W. El Camino Real, Suite #310, Mountain View, CA 94040. The Offer is not sponsored or administered by any other company.

Binding Agreement: In order to register for the Offer and to receive a Mastercard e-gift card, you must agree to the Rules. Therefore, please read these Rules prior to entry to ensure you understand and agree.

Eligibility: To be eligible to receive the offer, you must be at least twenty-two (22) years old, a legal resident of the United States, and have had an eligible Avéli treatment performed by a qualified medical provider or follow alternate entry rules below. The Offer is void where prohibited by law.

How to Enter and Redeem: Registering for the Offer is voluntary and does not require you to purchase anything from Sponsor. You can register for the Offer and redeem, in the following way:

1. On https://giftcard.myaveli.com/, register for the Offer by entering your name and email address. By submitting you agree to Avéli’s Terms and Conditions, Privacy Policy, The Naked Truth Patient Offer Official Rules, and for Avéli to contact you using the email provided.
2. Return to https://giftcard.myaveli.com/, to redeem your offer. Enter your name, email address, and phone number along with proof of treatment or if not deemed a candidate after a consult by a qualified provider, send your name, address, email address, phone number, participating provider’s name, address, phone number, date of consult, and proof of consult to the following address:

All redemptions must be submitted by Friday, November 15, 2024. You are limited to one (1) entry total.

Offer Period: The Offer begins on Thursday, September 5, 2024 at 12:00 AM PT, and ends on Thursday, October 31, 2024 at 11:59 PM PT (the “Offer Period”). All dates are subject to change.

Offer: All qualifying patients that register and request a redemption within the Offer Period, will receive a Mastercard e-gift card, valued at $250. The gift card will be sent via email to the address provided upon registration for the Offer from Avéli. Please allow 3-4 weeks for the gift card to be delivered and please check your spam folder.

General Conditions: All federal, state, and local laws and regulations apply, and Offer is void where prohibited by law. Sponsor reserves the right to disqualify any entry from the Offer if, in Sponsor’s sole discretion, it reasonably believes that the entry resulted from an attempt to undermine the legitimate operation of the Offer by cheating, deception, or other unfair playing practices or annoys, abuses, threatens, or harasses any other entrants or Sponsor. Entries using automated means are prohibited.  

Privacy: You agree that any personal data submitted with an entry, including name, email address, zip code, may be collected, processed, stored, and otherwise used by Sponsor and their affiliates for the purposes of conducting and administering the Offer. You also agree that any personal data submitted with an entry through the methods described in “How to Enter,” sections (a)-(b) above will also be used in connection with Sponsor’s promotional, marketing, and advertising activities. Without limitation, Sponsor may use your personal information to send you updates on Sponsor and Sponsor’s business, and promotional materials, from time to time. Warranty and Indemnity: By entering, and to the maximum extent permitted by law, you agree to release, defend, indemnify, and hold harmless Sponsor and its affiliates, employees, officers, directors, and agents from and against any and all claims, actions, suits, or proceedings, as well as any and all losses, liabilities, damages, costs, and expenses (including reasonable attorney fees) arising out of or relating to: (i) any misrepresentation you make in connection with the Offer; (ii) any non-compliance by you with these Rules; (iii) claims brought by persons or entities other than the parties to these Rules arising from or related to Entrant’s entry or involvement with the Offer; (iv) acceptance, possession, misuse or use of any prize or participation in any Offer-related activity or participation in the Offer; (v) any malfunction or other problem with Sponsor’s e-mail or computer systems in relation to the entry and participation in the Offer by you; (vi) any error in the collection, processing, or retention of entry in relation to the entry and participation in the Offer by you; or (vii) any typographical or other error in the registering or redemption in relation to the entry and participation in the Offer by you.  

Sponsor reserves the right, in its sole discretion and without prior notice, to suspend or cancel any Offer or alter the rules for any reason, including but not limited to alteration or corruption of the administration of the Offer by computer virus, technical problem, or other unforeseeable event.

Forum and Recourse to Judicial Procedures: These Rules shall be governed by, subject to, and construed in accordance with the laws of the State of California, United States of America, excluding all conflict of law rules. Any action relating to or arising from these Rules, or the Offer shall be commenced in the federal or state courts in Santa Clara County, California, and Sponsor, you, and Partner agree to waive any argument that venue there is not convenient or appropriate. If any provision(s) of these Rules are held to be invalid or unenforceable, all remaining provisions hereof will remain in full force and effect. To the extent permitted by law, the rights to litigate, seek injunctive relief or make any other recourse to judicial or any other procedure in case of disputes or claims resulting from or in connection with this Offer are hereby excluded, and Entrants expressly waive any and all such rights.

Avéli^® is indicated for long-term reduction in the appearance of cellulite in the buttocks and thigh areas of adult females as supported by clinical data demonstrating treatment benefits through one year of observation. Avéli is also indicated for soft tissue dissection during general and plastic surgical procedures. The most common side effects reported were mild pain within the first 24 hours and bruising and tenderness to the touch which typically resolved within 30 days. Some patients returned to normal activities within one day of the procedure and most within a week. As with any medical procedure, there are risks associated with the procedure.
